Notice:In applying for the recognition of certification,
the civil learning institution has to send the following
data, information by mail to the district concerned
unit. |

- A copy of this application form. ( Self-coping
version is permissible. Or pay a visit and download
at http¡G//www.hrd.gov.tw).
- A copying version of the registered document
which was approved by government authority
- A document which will include the regulations,
facilities, human resource distribution of the
applying institution.
- A copying version of the latest yearly business
income tax ( Except non-profit charitable institution
)
- A copy of Professional Personnel salary income
tax document (Copying version )
- The substantial training outcomes for the
past one year ( please raise up with its curriculum,
teacher, name-list of registration, roll call
, pictures---etc,.)
- A Future Plan of the Year ( Suggest to have
an explanation with some charts or lists which
will include the title of the course, its content,
brief introduction, classification and the time,
